If you’re starting to notice your jawline becoming softer or small jowls developing, you may be wondering:

Can CO2 laser tighten sagging jowls?

The answer is:

CO2 laser can improve mild skin laxity and may make early jowling look better, but it cannot completely lift significant sagging tissue.

Fractional CO2 resurfacing stimulates collagen remodeling and can improve the firmness, texture, and quality of the skin. Clinical studies have demonstrated measurable improvements in skin laxity after fractional CO2 treatment.

But jowls aren’t always caused by loose skin alone.

They can also develop because of:

  • Facial volume loss
  • Changes in facial fat
  • Descending tissue
  • Loss of structural support
  • Bone changes with aging
  • Skin laxity
  • Genetics

That distinction matters.

If the primary problem is mild loose skin, CO2 resurfacing may help.

If the problem is significant tissue descent or heavy jowling, a laser alone is unlikely to create the result you’re expecting.

The treatment has to match what is actually causing the jowl.

What Causes Sagging Jowls? #

Jowls develop as multiple layers of the face change with age.

It’s tempting to think:

“My skin is loose, so I just need to tighten the skin.”

But facial aging is more complicated.

The lower face is supported by:

  • Skin
  • Collagen
  • Fat compartments
  • Ligaments
  • Muscle
  • Bone

All of these structures change over time.

Collagen Decreases #

Collagen provides structural support to the skin.

As collagen decreases, skin can become:

  • Thinner
  • Less elastic
  • More crepey
  • Less firm

This can contribute to early laxity along the jawline.

Facial Volume Changes #

Facial fat doesn’t remain in exactly the same position throughout life.

Some areas lose volume while other areas may descend.

That can change the contour of the lower face.

Structural Support Changes #

Bone also changes with aging.

As facial support changes, the overlying soft tissue may begin to look less supported.

Gravity and Tissue Descent #

Over time, facial tissue can descend.

Once significant tissue descent has occurred, improving the skin alone may not fully correct the jowl.

How Does CO2 Laser Tighten Skin? #

CO2 laser resurfacing creates controlled microscopic injury in the skin.

This produces two important effects.

Immediate Tissue Response #

Ablative fractional CO2 creates microscopic columns of treatment.

Depending on the treatment parameters, this can produce some immediate tissue contraction.

Longer-Term Collagen Remodeling #

The controlled injury initiates wound healing.

Over the following weeks and months, collagen remodeling continues.

Research using objective measurements has demonstrated significant changes in skin mechanical properties following fractional CO2 resurfacing consistent with skin tightening.

This is why your final result shouldn’t be judged immediately after the laser.

Your skin can be finished peeling long before it is finished remodeling.

Is There Evidence That CO2 Laser Tightens Skin? #

Yes.

Several clinical studies have demonstrated improvements in skin laxity following fractional CO2 resurfacing.

One prospective study involving 45 patients reported approximately 50% mean improvement in skin laxity after a series of fractional CO2 treatments for photoaged skin.

Another study using objective measurements of skin elasticity found measurable changes consistent with tightening following fractional CO2 treatment.

There is also research specifically examining the neck and jowling.

In one study of 20 patients treated with fractional CO2 laser, researchers found significant improvements in skin laxity and jowling at both one month and one year after treatment.

Importantly, the authors noted that patients with moderate skin laxity and moderate jowling responded particularly well.

These studies demonstrate that fractional CO2 can have a genuine tightening effect.

But they do not mean that CO2 laser produces the same correction as surgery.

Can CO2 Laser Actually Lift Jowls? #

This is where expectations matter.

CO2 laser can tighten the skin.

That tightening may make early jowls look:

  • Less loose
  • More defined
  • Smoother
  • Slightly lifted
  • Better blended into the jawline

But CO2 laser isn’t physically repositioning the deeper tissues of your face.

If you have significant jowling because tissue has descended substantially, laser resurfacing can’t simply move that tissue back to where it was 15 years ago.

That’s a different problem.

Who Is a Good Candidate for CO2 Laser for Jowls? #

CO2 may be worth considering when you have:

  • Mild lower-face skin laxity
  • Early jowling
  • Crepey skin
  • Fine lines
  • Sun damage
  • Rough texture
  • Loss of skin firmness

It can be particularly attractive when you have multiple skin concerns at the same time.

For example, someone may have:

Mild jowling + fine lines + sun damage + rough texture + crepey skin.

CO2 resurfacing can potentially address several of those concerns during the same treatment plan.

When Is CO2 Laser Not Enough for Jowls? #

CO2 laser may not provide enough correction when you have:

  • Significant jowling
  • Heavy lower-face tissue
  • Advanced skin laxity
  • Significant tissue descent
  • Pronounced neck laxity
  • Large changes in facial structure

In those situations, resurfacing the skin may improve skin quality while leaving the underlying jowl largely present.

That’s why we don’t want to promise:

“CO2 laser will get rid of your jowls.”

Sometimes it won’t.

Can CoolPeel Tighten Jowls? #

Possibly—but expectations should be even more conservative.

The treatment intensity should match the concern.

What If My Jowls Are Caused by Volume Loss? #

This is where treating every jowl with a tightening device can go wrong.

Sometimes the lower face looks heavier because the upper and midface have lost structural support.

The answer isn’t necessarily to inject filler directly into the jowl.

Instead, the entire face needs to be evaluated.

Depending on facial anatomy, a patient may benefit from carefully selected structural support.

But adding too much filler in an attempt to “lift” significant jowls can also make the face look:

  • Heavy
  • Puffy
  • Wider
  • Overfilled

More filler doesn’t automatically create more lift.

Will Filler Fix Jowls? #

Sometimes carefully placed filler can improve the appearance of an early jowl by supporting adjacent areas or improving jawline transitions.

But filler has limitations.

If significant tissue has descended, continually adding filler can eventually create more volume without actually solving the sagging.

That’s why facial balancing requires restraint.

The goal isn’t:

“Where can we put more filler?”

The goal is:

“What is actually causing this change?”

What About Sculptra for Jowls? #

Sculptra is a biostimulatory injectable that encourages gradual collagen production.

It may be useful when the face is showing:

  • Collagen loss
  • Skin thinning
  • Early laxity
  • Generalized facial aging

Sculptra and CO2 laser address different aspects of aging.

Think of it this way:

CO2 laser ? resurfaces and remodels the skin

Sculptra ? supports gradual collagen production within treated areas

For some patients, improving both skin quality and collagen support may make more sense than relying on one treatment.

What About Radiesse for Sagging Skin? #

Hyperdilute Radiesse may also be considered as part of a collagen-stimulating strategy for appropriately selected patients.

It isn’t a surgical lift.

But it may be useful when the primary concern includes:

  • Skin quality
  • Collagen loss
  • Mild laxity
  • Crepey texture

Again, the right treatment depends on what is creating the appearance of the jowl.

CO2 Laser vs RF Microneedling for Jowls #

This is another common question.

Both can stimulate collagen, but they deliver energy differently.

CO2 Laser #

Works particularly well when laxity occurs alongside:

  • Sun damage
  • Fine lines
  • Rough texture
  • Crepey skin
  • Photoaging

Because CO2 resurfaces the skin, it can address multiple surface-aging concerns.

RF Microneedling #

Delivers radiofrequency energy into deeper layers through microneedles.

It may be useful when the primary concern is:

  • Mild-to-moderate laxity
  • Collagen loss
  • Texture
  • Lower-face or neck tightening

If someone’s skin surface already looks relatively healthy but their primary concern is deeper laxity, RF microneedling may make more sense.

If someone has significant sun damage, wrinkles, texture changes and laxity, CO2 may provide more comprehensive resurfacing.

Sometimes combination treatment may be appropriate.

Can You Combine CO2 With RF Microneedling? #

Potentially, but that doesn’t mean the treatments should automatically be performed together or close together.

They create different types of controlled injury.

Treatment timing should account for:

  • Skin type
  • Treatment intensity
  • Healing
  • Pigmentation risk
  • Desired downtime
  • Clinical goal

More treatments don’t automatically mean better results.

A thoughtful sequence is more important than stacking everything at once.

What About Weight Loss and Jowls? #

Weight changes can influence the lower face.

After significant weight loss, some patients notice more:

  • Facial volume loss
  • Skin laxity
  • Jowling
  • Neck laxity

If loose skin becomes more noticeable after weight loss, we need to determine whether the primary issue is:

Skin quality?

Volume loss?

Tissue descent?

Or a combination?

CO2 laser may improve the skin component.

It cannot replace substantial lost facial structure or surgically reposition descended tissue.

Does CO2 Laser Melt Jowl Fat? #

No.

CO2 resurfacing should not be viewed as a treatment designed to remove jowl fat.

Research evaluating fractional CO2 for neck rejuvenation found improvements in jowling and skin laxity, but the researchers specifically noted that fat deposition was not directly affected by the treatment.

The improvement in appearance was attributed primarily to the skin-tightening effect.

This distinction is important.

If the fullness you see is primarily fat rather than loose skin, resurfacing isn’t directly treating the underlying cause.

How Much Tightening Can I Expect? #

There isn’t one percentage that applies to everyone.

Results depend on:

  • Degree of laxity
  • Age
  • Skin quality
  • Treatment intensity
  • Treatment area
  • Number of treatments
  • Individual collagen response
  • Smoking
  • Sun exposure
  • Overall health
  • Aftercare

Published studies have shown measurable improvements in skin laxity, but those averages shouldn’t be interpreted as a guarantee for an individual patient.

Your before-and-after result will depend on where you’re starting.

When Will I See Tightening After CO2 Laser? #

Some early changes may become noticeable after the skin heals.

But collagen remodeling takes longer.

Improvement may continue developing over:

Several weeks ? early changes

2–3 months ? collagen remodeling becomes more noticeable

3–6 months ? continued remodeling

Some studies have demonstrated improvement persisting at one year following fractional CO2 treatment.

Don’t judge skin tightening after seven days.

How Many CO2 Treatments Do I Need for Jowls? #

There isn’t one universal number.

A more intensive treatment may produce meaningful improvement from one session.

A lighter approach may require a series.

But this is important:

If your jowling is too advanced for laser resurfacing to adequately address, performing more and more laser sessions isn’t necessarily the answer.

At some point, the limitation isn’t the number of treatments.

It’s the anatomy.

Can CO2 Laser Replace a Facelift? #

No.

This is probably the most important expectation to establish.

A facelift can surgically reposition deeper facial tissues and remove or redistribute excess skin.

CO2 laser cannot do that.

CO2 resurfacing improves the skin envelope.

Surgery can address deeper structural descent.

They are not equivalent treatments.

If someone has significant jowls and wants a dramatic change in the jawline, surgery may provide a level of correction that nonsurgical treatments cannot realistically reproduce.

What If I Don’t Want a Facelift? #

That’s completely reasonable.

Not everyone wants surgery.

But choosing nonsurgical treatment means accepting nonsurgical limitations.

For someone with mild-to-moderate aging who wants to:

  • Guarantee a sharp jawline

The best candidates tend to have mild-to-moderate skin laxity, especially when jowling occurs alongside concerns such as:

  • Fine lines
  • Crepey skin
  • Sun damage
  • Rough texture
  • Loss of firmness

The question isn’t simply:

“Can we tighten the jowls?”

It’s:

“Why are the jowls forming?”

Once we know that, we can choose a treatment that actually makes sense.
