If your acne has finally cleared but you’re still left with dents, uneven texture, or scars that show more in certain lighting, you may be wondering:
Can CO2 laser resurfacing actually remove acne scars?
CO2 laser resurfacing can significantly improve the appearance of certain acne scars, particularly atrophic or depressed acne scars. It works by resurfacing damaged skin and stimulating collagen remodeling beneath the surface.
But there is an important distinction:
CO2 laser can improve acne scars. It usually cannot completely erase them.
And not every acne scar should be treated with the same procedure.
The best results come from first identifying what type of acne scar you have and what is causing the depression in the skin.
How Does CO2 Laser Help Acne Scars? #
Acne scars can form when inflammation from acne damages collagen within the skin.

Rolling Scars #
Rolling scars create broad, shallow depressions that can make the skin look wavy or uneven.
Some rolling scars respond well to resurfacing.
However, many rolling scars are tethered underneath the skin by fibrous bands.
If the scar is tethered, resurfacing the top of the skin without releasing the attachment underneath may provide limited improvement.

- Shallower
- Smoother
- Softer around the edges
- Less shadowed
- Less noticeable in different lighting
The amount of improvement varies from patient to patient.
Why Do Acne Scars Look Worse in Certain Lighting? #
Acne scars often become much more noticeable under overhead or directional lighting.
Why?
Because depressed scars create shadows.
Even relatively small depressions can cast shadows that make the texture appear deeper.
This is why improving the transition between the scar and surrounding skin can make a meaningful visual difference even if the scar does not completely disappear.
The goal isn’t necessarily perfectly flat skin.
It is often to create smoother transitions and fewer visible shadows.
What Is Subcision for Acne Scars? #
Subcision treats acne scars differently from laser resurfacing.
A specialized needle or cannula is introduced beneath an appropriate depressed scar to release fibrous attachments that are pulling the skin downward.
Think of a tethered scar like a small area of skin being anchored from underneath.
You can resurface the top repeatedly.
But if the attachment remains underneath, the depression may remain.
Subcision addresses the tethering.
CO2 addresses the skin and scar remodeling.
For the right scar pattern, the two approaches can complement each other.
How Do I Know If My Acne Scars Need Subcision? #
Rolling scars that appear tethered may benefit from evaluation for subcision.
One clue can be how the scar behaves when the surrounding skin is stretched.
But scar assessment should be performed clinically because patients often have several scar types at the same time.
For example, one cheek might contain:
Rolling scars + boxcar scars + ice-pick scars.
That patient may need more than one treatment method.

These procedures are not really competitors.
They solve different problems.
CO2 Laser #
Primarily addresses:

If a scar is tethered, simply increasing the laser intensity may not solve the underlying problem.

What About PRF for Acne Scars? #
PRF may be used as part of a regenerative approach to support tissue healing and collagen-related treatment plans.
But PRF should not be viewed as a replacement for correcting the structural cause of a scar.

If significant surface irregularity is present, resurfacing may still be needed.

Others may benefit from a series of fractional treatments.

- Desired improvement
Combination treatment may also change how many laser sessions are needed.
When Will Acne Scars Improve After CO2? #
Don’t judge the final result immediately after your skin heals.

But collagen remodeling takes longer.
