If you’ve started noticing looser skin around your jawline, cheeks, eyes, or neck, you may be wondering whether a laser can tighten it without surgery.

The answer depends on how much sagging you actually have and what is causing it.

For mild skin laxity combined with wrinkles, sun damage, and rough texture, fractional CO2 laser resurfacing can be an excellent option because it stimulates remodeling while resurfacing damaged skin.

But if your primary concern is deeper skin laxity rather than surface damage, radiofrequency (RF) microneedling may sometimes be a better nonsurgical option because it delivers energy deeper into the dermis.

And if you have significant jowling or loose hanging skin, neither treatment should be presented as a replacement for surgery.

The best treatment isn’t simply the strongest machine.

The treatment has to match the concern.

Why Does Skin Begin to Sag? #

Skin doesn’t suddenly become loose because you reach a particular age.

Facial aging is a combination of changes occurring at different levels.

Over time, we experience changes in:

  • Elastin

  • Facial fat
  • Bone structure
  • Ligament support
  • Muscle
  • Sun damage

Collagen and elastin help provide firmness and resilience to the skin.

Fractional CO2 creates microscopic treatment zones in the skin.

The controlled injury initiates a wound-healing response and stimulates tissue remodeling.

This can improve several concerns simultaneously, including:

  • Fine lines
  • Wrinkles
  • Rough texture
  • Sun damage
  • Certain acne scars
  • Crepey texture
  • Mild skin laxity

This combination is what makes fractional CO2 particularly useful for patients whose skin isn’t only becoming loose—it also looks weathered, lined, textured, or sun-damaged.

How Does CO2 Laser Tighten Skin? #

CO2 laser resurfacing works through controlled ablation and thermal effects within the skin.

The treatment initiates a healing and remodeling response involving collagen.

Unlike a treatment that simply exfoliates the surface, fractional CO2 can produce remodeling that continues after the initial recovery period.

The goal is not to mechanically “pull” the skin upward.

Instead, we’re encouraging the skin to repair and remodel itself.

This distinction matters because the tightening from a laser is generally more subtle than what can be achieved surgically.

CoolPeel is generally used when we want lighter resurfacing with more manageable downtime.

  • Mild crepey skin

It can stimulate remodeling, but if skin tightening is your primary goal, a very superficial treatment may not provide enough correction.

Deeper Fractional CO2 #

A stronger fractional CO2 treatment may create more substantial remodeling.

It may be considered when there are:

  • More noticeable wrinkles
  • Greater textural damage
  • Advanced photoaging
  • Certain acne scars
  • More noticeable crepey skin
  • Mild laxity combined with surface aging

But greater treatment intensity usually means:

More downtime.

More redness.

More swelling.

More healing.

And potentially greater risk of complications.

Stronger isn’t automatically better.

Does CoolPeel Tighten Sagging Skin? #

CoolPeel may provide mild improvement in firmness and crepey texture through collagen remodeling.

But I wouldn’t describe CoolPeel as a nonsurgical facelift.

Its strengths are primarily improving:

  • Skin texture
  • Fine lines
  • Pores
  • Mild sun damage
  • Overall skin quality

If someone has significant jowls and asks for CoolPeel because they want their jawline dramatically lifted, expectations need to be corrected before treatment.

CoolPeel can make the skin look better.

That doesn’t mean it can reposition sagging facial tissue.

What About RF Microneedling for Sagging Skin? #

RF microneedling is another important treatment when skin laxity is the concern.

Unlike CO2 laser, RF microneedling uses small needles to deliver radiofrequency energy into targeted layers of the skin.

This creates controlled thermal injury within the dermis, stimulating collagen remodeling.

Clinical research supports RF microneedling for facial rejuvenation and skin laxity, including improvements in measures of wrinkles, dermal density, facial contour, and submental changes.

RF microneedling may be particularly useful for areas such as:

  • Lower face
  • Jawline
  • Neck
  • Cheeks
  • Periorbital area

For someone whose primary concern is firmness rather than surface sun damage, RF microneedling may sometimes make more sense than a superficial laser.

CO2 Laser vs RF Microneedling for Skin Tightening #

Neither treatment is universally better.

They have different strengths.

Fractional CO2 Laser #

Often stronger for:

  • Fine lines
  • Wrinkles
  • Sun damage
  • Rough texture
  • Acne scars
  • Surface resurfacing
  • Crepey skin

It can also stimulate collagen remodeling and improve mild laxity.

RF Microneedling #

Often considered when we want:

  • Deeper dermal remodeling
  • Mild skin laxity
  • Lower-face firmness
  • Neck rejuvenation
  • Texture improvement
  • Collagen stimulation

RF microneedling also avoids some of the epidermal resurfacing associated with ablative CO2, which can be an important consideration in certain skin types.

Sometimes the question isn’t:

“Which one is better?”

It’s:

“Which layer of the skin are we trying to treat?”

Which Is Better for the Neck? #

The neck is one of the areas where treatment selection becomes especially important.

Neck aging can include:

  • Crepey texture
  • Horizontal lines
  • Sun damage
  • Collagen loss
  • Skin laxity
  • Platysmal bands
  • Accumulated fat

Those are not all the same problem.

For crepey, sun-damaged neck skin, fractional CO2 resurfacing may be considered in appropriately selected patients.

For mild laxity and collagen loss, RF microneedling may be useful.

For significant loose skin, however, nonsurgical treatments may not provide the degree of correction someone expects.

What Is Best for Sagging Around the Jawline? #

Again, we first need to determine why the jawline is changing.

What looks like “loose skin” could actually involve:

  • Skin laxity
  • Facial volume changes
  • Fat distribution
  • Bone changes
  • Tissue descent
  • Muscle anatomy
  • Submental fullness

If the primary problem is mild skin laxity, an energy-based collagen treatment may help.

If the problem is significant tissue descent, repeatedly treating the skin may provide disappointing results.

You cannot laser every aging problem away.

Can CO2 Laser Tighten Under the Eyes? #

Fractional CO2 resurfacing can be particularly useful for appropriately selected patients with fine lines and crepey skin around the eyes.

This is an area where improving skin quality can create a noticeable rejuvenating effect.

The treatment may help soften:

However, under-eye aging can also involve:

Is CO2 Better Than Botox for Sagging Skin? #

They aren’t comparable treatments.

Botox primarily addresses muscle movement.

CO2 laser primarily addresses skin quality and remodeling.

Think of it this way:

Botox ? movement

CO2 laser ? skin quality

RF microneedling ? deeper collagen remodeling

Filler/biostimulators ? volume or structural support depending on the product

Surgery ? significant tissue repositioning

Once you understand which component of aging you’re treating, choosing a procedure becomes much easier.

What About Sculptra or Radiesse for Loose Skin? #

Sometimes what patients describe as “sagging” isn’t purely a skin problem.

Collagen loss and structural changes may be contributing to the appearance.

In selected patients, biostimulatory treatments such as Sculptra or hyperdilute Radiesse may be considered as part of a broader collagen-focused treatment plan.

These treatments aren’t lasers.

And they don’t perform the same job as resurfacing.

But they illustrate an important principle:

Looking younger isn’t always about treating the surface.

Can You Combine CO2 Laser and RF Microneedling? #

In some patients, a comprehensive treatment plan may use both technologies at different times.

For example:

CO2 laser ? improve surface damage, texture, wrinkles and photoaging

RF microneedling ? target deeper collagen remodeling and mild laxity

They don’t necessarily need to be performed together or at the same appointment.

The sequence and timing should be individualized.

More procedures don’t automatically produce better results.

Which Treatment Has More Downtime? #

Generally, more aggressive fractional CO2 resurfacing requires greater recovery than RF microneedling.

Following CO2 resurfacing, you may experience:

  • Redness
  • Swelling
  • Warmth
  • Dryness
  • Rough texture
  • Flaking
  • Temporary sensitivity

The recovery depends heavily on treatment intensity.

A lighter CoolPeel treatment usually has a more manageable recovery than deeper CO2 resurfacing.

RF microneedling commonly causes temporary redness and swelling, but because it does not resurface the epidermis in the same way as ablative CO2, recovery can be different.

What About Darker Skin Types? #

Skin type matters when choosing energy-based treatments.

Fractional CO2 can carry a risk of post-inflammatory hyperpigmentation (PIH), and this deserves particular consideration in patients with more melanin-rich skin.

Published comparisons of fractional CO2 and RF microneedling have found higher PIH rates with fractional CO2 in some treatment settings.

That doesn’t mean everyone with darker skin should automatically avoid CO2.

It means:

  • Patient selection matters
  • Treatment intensity matters
  • Pigmentation history matters
  • Sun exposure matters
  • Pre- and post-treatment care matters

Sometimes RF microneedling may offer an attractive alternative when we want collagen remodeling while minimizing certain pigment-related concerns.

What If You Have Melasma and Sagging Skin? #

This requires thoughtful treatment planning.

Melasma is not simply sun damage.

Because heat and inflammation may influence pigmentation, we don’t automatically reach for aggressive laser resurfacing simply because someone also wants tighter skin.

In a patient with melasma, the pigmentation history may influence which energy-based treatment we choose and how aggressively we treat.

The treatment has to match the entire skin—not just one concern.

How Many Treatments Are Needed to Tighten Skin? #

There is no universal number.

It depends on:

  • Degree of laxity
  • Age
  • Skin quality
  • Treatment technology
  • Treatment intensity
  • Area being treated
  • Previous treatments
  • Desired results
  • Tolerance for downtime

Some treatments work best as a series.

Others may be performed more aggressively with greater recovery.

The more important question isn’t how many sessions everyone needs.

It’s how much improvement can realistically be achieved without surgery.

When Will You See Skin Tightening? #

Collagen remodeling isn’t instantaneous.

You may notice early improvements in skin quality as the skin heals, but remodeling can continue over the following weeks and months.

This is particularly important when evaluating treatments designed to stimulate collagen.

Don’t judge the final result after one week.

Your skin needs time to respond.

Can a Laser Replace a Facelift? #

No.

This is one of the most important expectations to establish.

Lasers and other energy-based treatments can be excellent for:

  • Texture

But they do not surgically remove excess skin or reposition significantly descended facial tissues.

If you have significant:

  • Jowling

a surgical consultation may provide a more realistic discussion of what is required to achieve the result you want.

Choosing nonsurgical treatment is perfectly reasonable.

But expectations need to match what nonsurgical technology can actually accomplish.

So, What Is the Best Laser to Tighten Sagging Skin? #

For mild sagging combined with wrinkles, crepey texture, sun damage, and surface aging, fractional CO2 laser resurfacing can be an excellent option because it addresses both resurfacing and collagen remodeling.

If your primary concern is deeper mild skin laxity with less need for surface resurfacing, RF microneedling may sometimes be a better choice—even though technically it isn’t a laser.

For very early skin changes, a lighter treatment such as CoolPeel may provide subtle remodeling while improving skin quality.

For significant sagging, however, no laser should be marketed as equivalent to a facelift.

A simple way to think about it is:
